This saves you time. You can add or change later if necessary.
Payments do not have to be made all at once but per due dates for each sport chosen.

STEP 2
SUBMIT FORMS
Download & Complete:
- Sports Physical Exam Forms Valid for 1 year. Must submit new physical by expiration date.
- Health History Update Questionnaire Must submit before each sports season.
- Sports Physical Deferment Form Submit if physical expires during the season.
2 Ways to Submit completed Forms:
- Hand in original copy to Main Office
- Scan and email to athletics@acs-nj.org
